Social Prescribing Team
Social prescribing is an approach that connects people to activities, groups, and services in their community to meet the practical, social, and emotional needs that effects their health and wellbeing.
- Offers support for non-medical issues such as housing, benefits, and debt advice.
- Helps patients connect with local social and community support groups.
- Arrange monthly wellbeing cafes focusing on areas that are important to the local population these include frailty, fibromyalgia, endometriosis, menopause, and diabetes.

Care Co-ordinators
Our care co-ordinator team is a pivotal role with in the PCN and patient facing and helps provide support directly to patients and their cares around a wide number of areas these include:
- Cancer care support
- Learning disability
- Severe mental health (SMI)
- Frailty
- Dementia
- Childhood immunisations
- Carer support
- Signposting to community organisations

Physician Associate and Physician Associate Apprentice (Pas)
Our Physician Associates see patients from birth to end of life. They support and work under the supervision of a GP and alongside other professionals within the practice and wider support team.
- Taking medical histories from patients
- Performing physical examinations
- Diagnosing illnesses
- Reviewing patients with long term chronic conditions
- Develop management plans.
- Taking part in multi-disciplinary team meeting.
